I don’t really understand it, either, but I’m giving it an Arlo tag just because it’s McEldowney. The ones before and after this one are also themed The Thorax Clinic, but have a different patient.
Comics I Don't Understand 
Could be pure randomness. The young pianist married to one of the twins opened a concert with “Hold my calls.”

I’m at a complete loss. I’m not even sure what the “Thorax Clinic” is supposed to mean, even knowing the guy’s name is Thorax. It all sounds vaguely naughty, but mostly just vague.
If it weren’t for the continuity of art I would think these were panels from different comics, one with a punchline about farming and one with a setup about writing.
