This looks like an old Ziggy, perhaps from the early days of the strip. There was a time when it was considered inappropriate for conspicuously pregnant (human) women to be about in public. That time had pretty much passed by 1971, when Ziggy began syndication, but people still remembered it. The joke, of course, is that nobody has ever thought it necessary for pregnant cats to go into seclusion.
Usual John probably has it. But even conspicuously pregnant human women would be doing prenatal visits in a medical office. House calls were pretty much gone before 1971 for this type of thing.
Although … at a community fair last weekend, I saw a booth for a mobile medical practice built around house calls. I didn’t inquire further.

I think, since COVID, mobile health (i.e. house calls) has grown. Medicare will pay for both an in-office PCP and a mobile PCP.
