OK, so due to her first marriage she’s put the groom (from marriage #1?) into the side of the cake. But why take these feelings out on groom #2 on their wedding day?

She isn’t taking it out on Groom #2. When they cut the cake together they will slice the old groom. Joining together they will symbolically cut her ties with her first husband.
I am going to guess that at her first wedding, she fed her groom a bite of wedding cake with a fork, and he took a slice of wedding cake and smashed it into her face. So symbolically she is smashing his entire head and then some into the cake.
I think she is sending a message to groom number 2. “As I cut this cake see what I’m doing to my first husband’s head that is buried beneath my knife in the cake. Don’t be the next one. I may not wait for a cake.”
